New patients are encouraged to print and fill out the new patient forms prior to arriving at their appointment.
Lab Hours
Mon.-Fri., 7:30 a.m.-4 p.m.
Sat., 8 a.m.-12 p.m.
Lab services are available with or without an appointment. If you would like to schedule an appointment for lab services, 48 hours notice is required. Appointment blocks for lab services are Mon.-Fri., 12-3:45 p.m. and Sat., 10-11:45 a.m.
